- By Laws
- Secretary Cert
- Cert of Registration / Partnership Resolution
- DTI Registration
- Business Permit (for Sole proprietorship) w/ Valid ID
- Cert of Accreditation from DOLE (if employees union/ association)
- Copy of Financial Statements showing three (3) operating years.
- Endorsement Contract/Permit from LGU specific to area of interest. For barge suppliers, please also include Philippine Port Authority Permit to Operate
- DSO 18A Compliance
- Contractor’s Bond (if any)
